MISSED APPOINTMENTS, NO-SHOWS, OR LATE CANCELLATIONS

When you book an appointment, that time is reserved specifically for you. If you cannot make your scheduled appointment and wish to remain a client, please be respectful and cancel or reschedule your appointment at least 24 hours in advance.

If you fail to show for your initial intake appointment, the clinician may deny to reschedule your appointment.

POLICIES & SERVICES

Prescriptions will be written to last until your next appointment. It is recommended that you schedule your next appointment at the end of each session, so you are following your treatment plan and don't run out of medication.

Medication Refills We wanted to take a moment to remind you of our medication management policy. In order to ensure the safety and well-being of our patients, we require a thorough evaluation of your condition and a follow-up appointment to assess the effects of any medication changes. We understand that you may have concerns about your medication regimen and may wish to make changes. However, it is important to note that medication changes can have a significant impact on your health. Therefore, we ask that you schedule a follow-up appointment to discuss any concerns you may have. Please note that no medication changes will be made without a follow-up appointment. If you feel that you require a medication change before your next scheduled appointment, please contact our office to schedule an earlier visit.

Please allow up to 5 business days for refill requests to be approved. It's important to note that if you have not followed up with your agreed-upon treatment plan, refills may not be sent. It's expected that you will follow up to ensure the efficacy of your medication. Failure to follow up may result in dismissal from our services.

Additionally, certain medications require monitoring of labs. If you fail to get your labs drawn, your medication may be discontinued. We take medication management seriously and want to ensure that all our patients receive the highest quality care possible.

Controlled Substances such as benzodiazepines and stimulants, are strictly regulated by both the Drug Enforcement Administration and the Kentucky Board of Nursing/Pharmacy. These medications can be effective for certain conditions, but they require careful monitoring and must be a part of a comprehensive treatment plan that involves regular follow-up appointments. If you do not comply with the treatment plan, we will be unable to continue to prescribe these medications. Additionally, it's important to note that past or current use of a certain medication does not guarantee continued prescriptions.

It is important to emphasize that there will be no early refills of controlled substances under any circumstances, and no replacements will be issued for lost or stolen medications. If you miss a follow-up appointment, we will not be able to provide any refills of controlled medications. Random urine drug screenings may be required to ensure that medications are being used appropriately. We also advise that you store your medications in a safe and secure location, out of reach of children, to prevent any accidental ingestion or misuse.

Please note that medication diversion, which is the transfer of prescription drugs from the person for whom they are prescribed to another person or the illegal sale of prescription drugs, is a serious offense. Diversion of controlled substances not only puts individuals at risk for harmful effects but also carries legal consequences.

If there is any suspicion of medication diversion, the provider is required by law to report it to the appropriate authorities. In addition, misuse of controlled substances can lead to addiction, overdose, and other negative health outcomes. Therefore, it is crucial to follow the treatment plan agreed upon with your healthcare provider and to keep your medications safe from theft or misuse. It is also important to only use prescription medications as prescribed and not share them with others. Failure to follow these guidelines could lead to legal ramifications and harm to yourself or others and will lead to immediate dismissal from services.

Legal Matters/Disability: Please be aware that our practice does not participate in any legal matters, including but not limited to court evaluations or court testimonies. We do not provide legal opinions or perform psychiatric evaluations for legal purposes or disability determination. This includes child custody cases, workers’ compensation claims, or criminal cases. It is important to note that any requests for such services will be declined. Our focus is solely on providing mental health treatment and support to our clients. If you require legal assistance, we recommend seeking appropriate legal representation.

Additional Information: Our practice is committed to providing a safe and comfortable environment for all clients. We want to remind you that if you present to our offices and appear to be driving under the influence of alcohol or drugs, we reserve the right to report this to the police. This policy is in place to ensure the safety of others, including clients and staff members.

Additionally, we do not tolerate any form of threatening or violent behavior, or any behavior that is harassing or demanding in nature. If you engage in such behavior, you will be immediately dismissed from our services. We understand that seeking help can be a difficult and emotional process, but we must maintain a safe and respectful environment for everyone involved.

    • If you need help determining if your insurance is accepted, please call the office for assistance.

Please ensure that you inform our staff of any changes to your insurance coverage, including if you have more than one insurance plan. Please note that Medicaid cannot be billed as primary insurance, so if you have Medicaid along with another insurance plan, both insurances must be provided to us. Additionally, please keep in mind that while we do accept insurance, it is ultimately your responsibility to ensure that we are in network with your insurance provider, as well as to determine your copay and deductible amounts. You will be responsible for any charges that are not covered by your insurance, including cases where the insurance coverage falls short of the entire bill. It is advisable to contact your insurance provider prior to scheduling an appointment to confirm coverage and avoid unexpected expenses.

Billing Info & Fees

  • Your copay or portion of your deductible is due at the time of service unless arrangements have been made prior to your appointment.
  • If your account balance is over $150 and/or no payments have been made for more than 60 days, the provider may not continue to provide services until a payment has been made or arranged.
  • There is a $30.00 fee for checks written with insufficient funds.
  • Balances past due for 90 days may be sent to an outside collections’ agency.
  • There is a $25 fee for FMLA forms to be filled out and will be done at the discretion of the provider.
